Dr. Oz is looking for his biggest fans to attend a special show all about YOU—the viewers. On this show, viewers get to choose what topics they want to see, which favorite “assistant of the day” to bring back, and even what giveaways they want.

How can you enter? Videotape yourself telling us why you think you are Dr. Oz’s biggest fan in 60 seconds or less, then upload your video in the form below.

What are we looking for? Be as creative, enthusiastic and as original as possible! Have fun with it. Show us what a dedicated viewer you are to “The Dr. Oz Show.” And remember: it’s a no-embarrassment zone!

What do I get if I win? If you are selected to be Dr. Oz’s biggest fan, you will receive a free trip to New York City, stay in a hotel for the night, and then see Dr. Oz up close and personal for a live studio taping of “The Dr. Oz Show” on October 27th.

How long does the contest run? The contest begins Thursday, October 14th and ends on Wednesday, October 20th so you need to act now!

How will I find out if I won? We will notify the winners on Thursday, October 21st. You need to be available to travel on October 26th and attend the taping on October 27th.

How do I upload my video?

• After you make your video, click on the BROWSE button to select the video file from where it is stored on your computer.

(video formats accepted are .flv, .mpg, H.264, .avi, .wmv, MOBILE .3gp, 3g2 and HD H.264, MPEG-4, and .wmv)

• Next, type a title for your clip or photo.
• Then, in the ‘Author’ field, type your name and the city that you’re from.
• Add a description and your email address.
• After you read and agree to the Terms of Use, check the box and click on SUBMIT.
